It’s never too early to start planning—Advanced Care Planning (ACP) is an ongoing process of preparing for your future health and personal care. Informed by personal beliefs, values and healthcare preferences, ACP ensures that your loved ones and healthcare team act in your best interests based on your prior expressed wishes, in the event that you lose the capacity to advocate for your own healthcare.

 

Let Senior Medical Social Worker, Nel Lim, guide you through the importance of ACP and how to kickstart the conversation with your healthcare providers and loved ones in this informative talk.

Nel Lim is a dedicated Senior Medical Social Worker with over more a decade of experience serving the community. With a strong passion for elder care and advocacy, Nel has spent the past years supporting older adults through meaningful engagement, programs and clinical interventions. Recognising the importance of future planning, Nel has previously created groupwork and programs aimed at raising awareness of Advanced Care Planning (ACP) among the elderly. The work continues to empower seniors to make informed decisions about their health and well-being.
